What this website collects

This site does not use advertising trackers, third-party analytics, or cookies for marketing. It sets no cookies of its own. Fonts and images are served from our own domain rather than a third-party network, so browsing these pages does not report your visit to anyone else.

Our hosting provider, Cloudflare, records standard server request logs (including IP address and browser type) for security and performance purposes. These logs are retained by Cloudflare under their own policy and are not used by us to identify individual visitors.

The contact form

If you complete our contact form, we receive the name, phone number, email address, location, and reason you provide, and use them only to respond to your enquiry. The message is delivered by email to the Family Care office you selected.

Please do not send medical information through this form. Email is not a secure channel, and our contact form is not covered by the safeguards that protect your medical record. If you need to discuss a diagnosis, treatment, or any other health detail, call the office directly and we will speak with you privately.

Your medical information

Health information collected in the course of providing care is governed by HIPAA, not by this website policy. Family Care maintains a separate Notice of Privacy Practices describing how we may use and disclose your protected health information, and your rights regarding it.
